Template letter for NHSP follow up

PHE has released a letter for audiology departments to use for screen referrals during Covid-19 if they are unable to offer an assessment appointment within 4 weeks due to service/clinic closures.
It will also be available in S4H for screening services to download and share with their audiology service.
Public Health England has asked that you please remember to include the audiology department telephone number in your letter headers.
